아스피린의 암 예방 효과

결장암 및 심장질환의 예방에 셀레브렉스(celebrex)보다 아스피린(aspirin)이 더욱 효과가 있다는 연구 결과가 발표되었다.

셀레브렉스나 아스피린은 둘 다 염증작용 효소인 사이클로옥시게나제-2( cyclooxygenase-2 : COX-2)를 억제하는 작용을 하는데 특히 셀레브렉스는 COX-2의 작용을 선택적으로 억제하는 한편, 아스피린은 COX-2를 포함하여 다른 관련 효소의 작용도 억제하는 것으로 알려져 있다.

최근 다수의 건강한 사람들이 암 및 심장질환을 예방하기 위한 목적으로 아스피린을 복용하고 있다. 한편 셀레브렉스와 같이 COX-2의 작용을 선택적으로 억제하는 약물은 결장암의 예방효과는 있지만 심장질환에 대한 효과는 확실하지 않은 것으로 알려져 있어 미국 매사추세츠 종합병원의 Chin Hur 박사 연구진은 이러한 사실을 조사하여 그 결과를 암 학회지(Medical Journal of Cancer)에 발표하였다.

연구진은 컴퓨터 시뮬레이션을 통해 프로그램에서 건강한 50세의 사람들을 대상으로 하루 한 알의 아스피린이나 하루 두 알의 셀레브렉스를 투여하였다.

연구진은 두 약물이 모두 결장암의 예방효과가 있을 것으로 추측하였고 그 외에 이러한 약물 사용에 따른 합병증(complications) 및 기타 위험 요인을 조사하였다. 연구 결과 10년 동안 아스피린을 사용했을 경우 셀레브렉스를 사용했을 경우보다 $23,000 이 절약되고 그 효과 또한 더 좋은 것으로 나타났다.

셀레브렉스를 복용한 그룹은 아스피린을 복용한 그룹보다 합병증이 3.9% 더 많았고 사망률 또한 0.17% 더 증가한 것으로 나타났는데 이는 셀레브렉스를 복용한 그룹의 26명 중 1명이 합병증을 가졌고 588명중 1명이 사망했음을 의미하는 것이다.

연구진은 향후 이러한 약물들이 얼마나 암 예방에 효과적인지를 밝히기 위한 연구가 필요하다고 언급하였다. 하지만 이번 연구 결과 아스피린이 암 및 심장질환의 예방에 더욱 효과가 있다는 사실은 확실한 것으로 나타났다고 덧붙였다.

NEW YORK (Reuters Health) - Aspirin is a better choice than Celebrex (celecoxib) for people wishing to lower their risk of colon cancer as well as heart disease, findings from a new study suggest.

Both drugs work by blocking an enzyme called cyclooxygenase-2 or COX-2. However, Celebrex is considered a "selective" blocker because it only affects COX-2, whereas aspirin also affects related enzymes.

"Our analysis suggests that average-risk individuals contemplating COX-2 inhibitor therapy for colon cancer prevention should keep in the mind the added cardiac benefit of aspirin," lead investigator Dr. Chin Hur told Reuters Health.

Hur, of Massachusetts General Hospital, Boston, and colleagues note in the medical journal Cancer that there is a growing trend for healthy people to use aspirin to prevent both cancer and heart disease. Selective COX-2 inhibitors may also be protective against colon cancer, but may not provide heart benefits.

To investigate, the researchers constructed a computer model involving simulated groups of healthy men aged 50 years. These hypothetical subjects took either aspirin daily or Celebrex twice daily.

Both agents were assumed to have an equivalent effect on colon cancer risk, but data from the literature and other sources were used to assess the risk of complications and other events.

Over a 10-year period, compared to Celebrex, aspirin cost $23,000 less and resulted in an improvement in quality of life.

The Celebrex group had 3.9 percent more complications and 0.17 percent more deaths. This amounted to 1 complication for every 26 patients treated and 1 death for every 588 patients.

The researchers concede that further studies are needed to establish how effective these agents are in preventing tumors. However, the results suggest that in the circumstances studied "aspirin is the clear choice," they add.
